Thank you for your payment to : Elite Brain and Body management IMPORTANT: * CAN be used by itself or COMBIENED with PEMF , H-Wave therapy, NIR (HELMET) Photo biomodulation, Neurofeedback or Neuromodulation for added cost. ADD ON services available for extra charge. * You will be charged for FULL SESSION booked without 24 hours prior notice of cancelling appointment. ** This includes sessions booked in packages, at ONE session forfeited from package. EXAMPLE: Package of 20 sessions purchased at ( Dollar amount). Missed appointment without 24 hours prior notice given, ONE session will be deducted from session account. EX: ( 20 - 1= 19) *** Sessions purchased in packages are evenly divided into dollar amount per session at a discounted rate as a thank you. (Any request for reimbursement of unused package sessions, the used sessions MAY BE CHARGED AT FULL SESSION PRICING, and reimbursement will be given as SESSION/SESSIONS USED, AND/OR ANY SESSION NOT CANCELLED WITHOUT 24 HOURS PRIOR NOTICE *charged at full price MINUS/ LESS THAN dollar amount left for PRE-PAID package/packages ( TOTAL PACKAGE COST).
